COVID-19 Screening Questions
Now that we are open again, we have some new guidelines set out by the Ministry of Health for our clinics.
We require all our staff and customers to wear a mask when in our offices, as well as use hand sanitizer provided. Here are some screening questions you can expect to be asked before coming in for an appointment.
-Have you or anyone in your family recently traveled outside of Canada in the past 14 days?
-Have you or anyone you’ve had contact with tested positive for COVID-19?
-Do you have any of the following symptoms? :
-New onset cough
– Worsening chronic cough
-Shortness of breath
-Decrease of loss of sense of taste or smell
-Unexplained fatigue, malaise, muscle aches
-Nausea, vomiting, diarrhea or abdominal pain
-Pink eye ( conjunctivitis)
-Runny nose or nasal congestion without other known cause
-If you are 70 years of age or older, are you experiencing any of the following symptoms: Delirium, unexplained or increased number of falls, Acute functional decline, Worsening of chronic conditions?
If you have any of these symptoms please do not come into the office, reschedule your appointment and seek medical help.
